SUMMARY
The Health Information Specialist contributes to effective operation of Health Information Services to meet the needs of internal and external customers by performing specialized tasks related to birth certificate processing, deficiency analysis, document imaging, or reconciliation of discharged records.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.
Must be able to retrieve patient clinical and demographic information from existing legacy systems and multiple media sources including, paper, fiche, microfilm, standalone CD’s, on-line systems for historical information created prior to implementation of electronic record.
Ability to work in electronic systems which may include, but are not limited to : Meditech and Excellian.
Differentiate location of patient clinical information (i.e. paper vs. electronic environment) and appropriate document routing/management to each system.
Assist providers in completion of incomplete medical records which may exist in both paper and electronic formats, pulling requested records, updating chart deficiencies, ensuring all episodes are updated as they are completed.
Performs quality checks on all documentation before it enters the electronic environment. This also consists of checking for unsigned documentation and adding deficiencies.
Enter all birth certificate data in the MRC program to transmit to the state, provide parents a complimentary birth certificate along with proof of applying for a social security card.
Collection of outpatient selected indicators for proper documentation and JCAHO requirements along with attendance at the Medical Record Committee.
Knowledge of regulatory requirements including medical staff rules and regulations, Critical Access, JCAHO in order to assist physicians who have questions about chart completion requirements. Create a readily available list for the physicians incomplete records and working with them to keep the patient records current.
Electronically route or mail out incomplete records for physician completion.
Knowledge of HIPAA privacy requirements related to access and disclosure of PHI to authorized individuals, statutory requirements related to research releases.
Supports the policies and procedures of the organization and the standards of the Joint Commission as required for full accreditation.

EDUCATION and/or EXPERIENCE
Medical Secretary training/certificate or RHIT credentials preferred; medical terminology, thorough understanding of patient records/charts required.
LANGUAGE SKILLS
Ability to read and comprehend simple instructions, short correspondence, and memos. Ability to write simple correspondence. Ability to effectively present information in one-on-one and small group situations to department managers, patients, and co-workers.
MATHEMATICAL SKILLS
Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent and to draw and interpret bar graphs.
REASONING ABILITY
Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations.
CERTIFICATES
Medical Secretary Certificate from an accredited school or RHIT Credentialed.
